Remove from service if:

  • Kinking, crushing, birdcaging, or other distortion of the rope structure
  • Severe corrosion, heat damage, or cracked/deformed/worn end attachments

Remove from service if:
  • Cracks or breaks anywhere in the chain or fittings
  • Excessive wear, nicks, or gouges (a nick deep enough to catch a fingernail is cause for concern)

  • Holes, tears, cuts, snags, or excessive abrasive wear

Shackle · per ASME B30.26
Remove from service if:
  • Cracks, bending, or visible damage on the shackle body
  • Wear or distortion that reduces the original dimension beyond the allowance
  • Pins seized, cross-threaded, or not seating fully; worn threads
  • A substituted or non-matching pin — only the correct manufacturer pin
  • Missing or illegible working-load-limit markings
Hook · per ASME B30.10
Remove from service if:
  • Throat opening increased beyond the manufacturer/standard allowance
  • Twist from the plane of the unbent hook beyond the allowance
  • Wear at the bearing point or saddle beyond the allowance
  • Cracks, sharp nicks, or gouges; a latch that no longer functions where required
  • Missing or illegible identification

Rigging gear inspection fails in a specific, repeatable way: the crew does the daily visual check religiously, and everyone assumes that's the program. It isn't. The standards layer a recorded periodic inspection on top of the daily check, and they require every piece to be individually identified — because you can't manage what you can't tell apart. Three inspections, not one: initial, frequent, periodic

The most common compliance gap in rigging isn't skipping inspections — it's running only one of the three the standards require and thinking the job's done. OSHA 1910.184 and 1926.251, together with ASME B30.9, define a layered program, and each layer catches something the others miss.

Initial inspection

Performed by a qualified person before any new, repaired, altered, or modified gear is first put into service — confirming the item matches its tag, its rated capacity, and its intended use. This is the baseline the item's whole record starts from.

Frequent inspection

A visual and hands-on check by a competent person each day or shift the gear is used, plus during use where conditions warrant. No written record is required — but a distant glance doesn't count; it's a close, tactile inspection of the sling and all fastenings.

Periodic inspection

A documented inspection with a written record, by a designated competent or qualified person, at intervals set by service severity. This is the layer most often missed — and the one an auditor asks to see, because it's the only one that leaves proof.

How often the periodic inspection falls is driven by service classification: normal service (routine use at or below rated load) typically runs to a yearly documented inspection; severe service (chemical exposure, high heat, abrasive environments) tightens to monthly-to-quarterly; and special service is set by a qualified person. You can't inspect what you can't identify

Here's the problem unique to rigging: the gear all looks alike. Two identical-looking web slings on the same rack can have wildly different histories — one three months old, one condemned last week and somehow still in the pile. The standards address this by making identification mandatory: a sling without legible identification markings can't legally be used, and the same principle runs through shackles and hooks, which carry their working load limit as a permanent marking.

Quarantine: the step between "failed" and "gone"

Finding a condemned sling is only half the job. The dangerous gap is between the moment a piece fails inspection and the moment it's physically incapable of going back on a hook — because in that gap, a failed sling looks exactly like a good one on the rack. Most dropped-load incidents involving old gear trace back to something that was known to be bad but never removed from circulation.

A defensible program closes that gap deliberately: the instant an item meets a removal criterion, its status flips to out-of-service, it's physically segregated from the usable gear, and — for genuinely condemned items — it's destroyed so it can't be salvaged (chain slings, for example, are often cut into short lengths and recycled). The record has to reflect that journey: found, quarantined, and either repaired-and-recertified by a qualified person or destroyed. Other visible damage that raises doubt about continued safe use is also cause for removal. What are the removal criteria for a hook and a shackle?

Hooks are governed by ASME B30.10 and shackles by ASME B30.26. A hook is generally removed from service when the throat opening has increased beyond the manufacturer's or standard's allowance, when it is twisted beyond the allowed limit from the plane of the unbent hook, when wear at the bearing point or saddle exceeds the allowance, when there are cracks, sharp nicks, or gouges, when a required latch no longer functions, or when identification is missing or illegible. A shackle is removed when the body shows cracks, bending, or visible damage; when wear or distortion reduces original dimensions beyond the allowance; when the pin is seized, cross-threaded, or does not seat fully, or has worn threads; when a substituted or non-matching pin has been used instead of the correct manufacturer pin; or when the working-load-limit markings are missing or illegible. Because the exact numerical allowances vary by size, type, and manufacturer, always measure against the governing standard and the manufacturer's specification for the specific item rather than a generic figure.

Why does every sling and shackle need its own identification?

Identification is both a legal requirement and the practical foundation of any rigging program. Under the standards, a sling without legible identification markings must not be used, and shackles and hooks must carry permanently affixed, legible markings including their working load limit. The reason is that rigging gear is nearly impossible to manage otherwise: two identical-looking slings can have completely different ages, histories, and conditions, and without a unique identity you cannot tell which one was inspected, which one was condemned, or which one was actually on the hook during an incident. Unique identification turns each piece into a trackable asset with its own record — rated capacity, certification, inspection history, and current status. It is also what makes a quarantine meaningful: a condemned item's out-of-service status can only follow the item if the item can be told apart from every other one on the rack. When an auditor or an investigator asks for the history of a specific piece of gear, that individual identity is the only thing that can produce an answer.

What should happen to rigging gear that fails inspection?

Gear that meets a removal-from-service criterion must be taken out of use immediately, and a defensible program does not stop at "noticed it's bad." The item's status should be changed to out-of-service on its record, and it should be physically segregated from the usable gear so it cannot be picked up for a lift — the dangerous gap is the period where a failed sling still sits on the rack looking like a good one. From there, the item is either repaired and recertified by a qualified person (repairs must be done by the manufacturer or a qualified person, and the item re-inspected before returning to service) or, if it is genuinely condemned, destroyed so it cannot be salvaged and returned to circulation; chain slings, for instance, are commonly cut into short lengths before disposal. The record should capture that full journey — found, quarantined, and resolved — so there is proof the item was handled correctly.
